Children's Faith Formation

You can fill out the form and return it to the parish office along with registration payment during business hours or through the mail slot in the office door.
              Payments are as follows:
                   -$40.00 for Non-Sacrament Years
                   -$80.00 for First Holy Communion and Confirmation Years

For children celebrating the Sacraments of First Holy Communion (3rd grade) and/or Confirmation (7th grade), please bring us a copy of their Birth Certificate, Baptismal Certificate and Holy Communion Certificate. You can also scan and email a copy to: [email protected] and I can place it in their file.
